This website is for business discovery, pilot recruitment, and general inquiries. It is not a patient portal or clinical system. This notice does not serve as a HIPAA Notice of Privacy Practices.

2. Information we collect

When you submit the founding-pilot application, we may collect your applicant type, name, organization, role, email address, phone number, city, state, workflow interests, consent confirmation, submission time, and a reference number. Our systems may also process limited technical information needed to operate and protect the website, such as request, device, browser, and security-log information.

3. Information we prohibit

Do not provide patient names, dates of birth, medications, prescriptions, diagnoses, insurance information, medical-record information, payment-card information, or other PHI. If prohibited information is submitted accidentally, contact us promptly so we can evaluate and remove it where appropriate.

4. How we use information

• Review and respond to pilot or partnership interest.
• Understand non-clinical workflow needs.
• Operate, secure, troubleshoot, and improve the website.
• Maintain records of consent and prevent misuse.

5. Sharing

We do not sell personal information. We may disclose information to service providers that support website hosting, security, storage, and communications; to professional advisers; when legally required; or to protect Presendia Health, visitors, and others. Providers should receive only the information reasonably necessary for their work.

We retain information only as reasonably needed for the purposes above, legal obligations, dispute resolution, and security. We use administrative and technical safeguards appropriate to this discovery-stage website, but no internet system can be guaranteed completely secure.

7. Your choices

You may ask to access, correct, or delete information you submitted, or withdraw from pilot communications, by emailing privacy@presendiahealth.com. We may need to verify your request and may retain information when legally permitted or required. Marketing messages, if introduced, will include an unsubscribe method.

8. Cookies and analytics

We do not currently use advertising cookies or sell information for targeted advertising. Essential technologies may be used to deliver and secure the site. This notice will be updated before materially different analytics or advertising technologies are introduced.

9. Children

This business-recruitment website is not directed to children under 13, and we do not knowingly solicit their personal information. Patient-interest applicants must be at least 18 years old.

10. Changes and contact

We may update this notice as the service changes. A revised effective date will appear above. Privacy questions and incidents may be reported to privacy@presendiahealth.com.
